目的探讨乙状窦还纳术对乙状窦憩室或骨壁缺损引起搏动性耳鸣的疗效,并分析特殊病例特点及原因。方法回顾性分析35例乙状窦还纳术治疗搏动性耳鸣患者临床资料,在术前、术后1月、3月、6月进行耳鸣量表THI评分及统计分析。结果无效0例、显效18例、痊愈16例、失访1例。显效组THI量表术前:65.78±5.40,术后6月:28.11±4.52,统计学差异显著。结论对乙状窦憩室或骨壁缺损引起的搏动性耳鸣进行乙状窦还纳术疗效确切,局麻手术有利于进行术中评估。
Abstract：Objective To report efficacy of sigmoid sinus surgery in the treatment of pulsatile tinnitus caused by sigmoid sinus diverticulum or bone wall defect, and characteristics and etiologies in unique cases. Methods Clinical data of 35 patients with pulsatile tinnitus treated by sigmoid sinus reduction were analyzed retrospectively. The Tinnitus Handicap Index(THI) scores were before, 1 month, 3 months and 6 months after operation were statistically analyzed.Results Pulsatile tinnitus markedly improved in 18 cases and completely resolved in 16 cases. One case was lost to follow-up. The THI scale improved from 65.78 ±5.40 before operation to 28.11 ±4.52 at 6 months after operation. Conclusion Sigmoid sinus reduction is effective for pulsatile tinnitus caused by sigmoid diverticulum or bone wall defect, and local anesthesia is beneficial for intraoperative tinnitus evaluation.
