目的 调查年龄≥50岁人群的眩晕发病情况与认知功能相关性，为防治中老年人群眩晕制定策略提供基础。方法 对成都5个社区年龄≥50岁人群，符合调查标准的对象，采用问卷调查以及床旁检查的方式，发放问卷300份。结果 回收有效量表284份(应答率94.67%)，男100例(35.2%)、女184例(64.8%)。所有调查对象中认知功能下降的患者有244例(85.9%)；自诉有前庭症状的69例患者中，认知功能得分下降的有60例(91.3%)。结论前庭综合征在目标人群中较常见，认知功能下降在其中普遍存在。前庭症状有5组之间的认知功能下降检出率存在显著差异(χ2=11.308,P=0.01)，前庭功能床旁检查阳性组和阴性组存在显著差异(χ2=13.775,P=0.003)，阳性组、有前庭症状组检出率最高。年龄、学历程度、前庭功能检查和认知功能下降具有显著相关性(OR分别为1.055、0.530、2.235)，且学历程度是认知功能的保护因素，年龄和前庭功能异常都是其危险因素。
Abstract：Objective To investigate the correlation between vertigo and cognitive function to provide a basis for developing strategies for vertigo prevention in middle-aged and elderly people. Methods Questionnaires were distributed to 300 people aged 50 years in five communities in Chengdu who met inclusion criteria. Results A total of 284 valid returns(94.67%) were collected, including 100 men(35.2%) and 184 women(64.8%). Among the valid returns, 244 patients(85.9%) reported cognitive decline and 69 patients reported vestibular symptoms, including 60(91.3%) showing decreased cognitive function scores. The rate of cognitive decline was higher in patients with vestibular symptoms than in those without(χ2=11.308,P=0.01), and in patients with positive bedside exams than in those with negative exams(χ2=13.775,P=0.003). Age, education and vestibular function were correlated with cognitive function(OR=1.055,0.530, and2.235), Conclusion Vestibular syndrome is relatively common in this target population with prevalent cognitive decline. While level of education may be a protective factor against cognitive decline, age and vestibular function abnormalities both appear to be risk factors.
