关于机械臂辅助下的耳内镜鼓膜成形术可行性的评价研究

目的观察、评价在机械臂辅助下的耳内镜鼓膜成形术的疗效。方法对2015年08月-2017年01月因132例(132耳)慢性中耳炎陈旧性鼓膜穿孔在我院行手术治疗的患者进行研究。其中64例(A组)接受了机械臂辅助下的耳内镜鼓膜成形术,另外68例(B组)行常规耳内镜下鼓膜成形术,所有病例均符合鼓膜成形的手术指征,由我科同一组医师在相同的手术环境下完成,比较手术用时和术后鼓膜的一次愈合情况。结果 A组术后鼓膜一次性愈合情况共59耳(92.18%),B组为62耳(91.17%),差异无统计学意义(X2=0.044,P=0.834);A组平均手术用时(35.5±4.47 min),B组为(39.0±4.83 min),差异具有统计学意义(P<0.001)。结论机械臂辅助下耳内窥镜鼓膜成形术治愈率与常规耳内镜鼓膜成形术无明显差距,手术用时略微缩短,但其术野清晰、稳定,解放左手,便于双手收操作等优点值得推广。

Abstract:

Objective To report results of mechanical arm aided endoscopic myringoplasty. Methods A study was conducted on 132 cases treated from August 2015 to Jan 2017 in our hospital for chronic otitis media and tympanic membrane perforation. Among them, 64(Group A) underwent endoscopic myringoplasty with the aid of a mechanical arm, while the other 68(Group B) received conventional endoscopic myringoplasty. The indication for myringoplasty was confirmed in all cases, and all procedures were completed by the same group of surgeons in the same operation setting. Results Good tympanic membrane healing was seen in 59 cases(92.18%) in Group A and in 62 cases(91.17%)in Group B(X2=0.044, P=0.834). The average operation time was 35.5±4.47 min in Group A and 39.0±4.83 min Group B(P<0.001). Conclusion Mechanical arm aide endoscopic myringoplasty and conventional endoscopic myringoplasty show no significant gaps, although the former requires slightly shorter operation time with more clear and stable view of the operation field. The mechanical arm can also free the left hand, allowing double hand operation, making its application more appealing.
